The role of the Community Health Team is to increase access to health and social services and to facilitate the community's capacity to overcome barriers to wellness. Programs designed to overcome such barriers focus on issues such as nutrition, seniors, mental heatlh, community development, homeless outreach, case management, cultural groups, housing and the environment.
