Asthma

RPCHC participates in the Primary Care Asthma Program (PCAP). This is an evidenced based program that provides primary care providers (doctors, nurses and nurse practitioners) with decision aids to support best practice regarding asthma assessment, diagnosis and management. PCAP is funded through the MoHLTC and is in 12 care sites across Ontario.
The Primary Care Asthma Program is delivered within a multidisciplinary team of primary care providers with the support of a registered respiratory therapist. RPCHC clients can be referred for respiratory assessment, lung function testing, education, self-management skills and smoking cessation support. Clients are also seen for assessment of COPD (chronic obstructive pulmonary disease) and other respiratory related concerns. Referrals can be made through either a doctor or nurse practitioner at the Regent Park Community Health Centre.
